 Medical and Parent Permission forms are required for students to attend. Please bring the completed forms with you. The medical form must be on a single piece of paper (printed back to back) and include a copy of the student's insurance card. Parent Permission Medical Permission

• Earn a free weekend trip (Any student attending wordless book training for the first time is eligible to go to the statewide CYIA retreat on March 21-23 for free!)
