Before you begin
Because you volunteered with CYIA last year, we already have a lot of your information on file so some of the application steps do not need to be repeated. However, there are some things that we are required to update annually so please complete each of the steps below in order to ensure that all of your training and information is up to date.
Refresh Steps
- Watch Protecting Today’s Child
We are required by law to have all of our volunteers both new and returning watch the following presentation in order to ensure that they are familiar with the content. The presentation can be watched here.
- Medical Liability and Release Form
This form will need to be completed in the presence of a parent/guardian. Please make sure that the entire form is printed on one sheet of paper with the two pages back to front. This form allows our staff to help your student in case they require immediate medical attention.
- Parental Activity Permission Form
This form will need to be completed in the presence of a parent/guardian. This form must be signed by your parent/guardian and gives you permission to participate in any events related to CYIA.
- Sign the Worker’s Compliance Agreement
This form must be completed annually by returning students. Before signing the form, please be sure to have read the statement of faith, reviewed our chapter CYIA policies, and watched the Protecting Today’s Child presentation.
- Returning Student Application
This form helps us update contact info and organize your paperwork for the upcoming year. This form requires you to certify that you have completed the steps above so don't fill it out until you are ready.
- Get Ready for Training
You are all done filling out forms and we look forward to seeing you at our upcoming training sessions! Be sure to bring any signed forms with you to your first training session. You are now ready to move on to “Phase 2”.
